All six physicians are accepting new patients at this time. All doctors see patients in our Hinsdale office; Drs. Sweet, Madej, Schneiderman and Shah see patients in our Bolingbrook office as well. We work hard to try to ensure all patients are seen within a week's time.

After registering and setting up your appointment time, the secretaries will discuss with you the necessary documents needed to ensure that your consultation with the doctor is complete. It is imperative that these records be available for the doctor on the day of your appointment. If you are bringing in records, please be sure they are copies we can keep in our office (there is a $10 fee for the copying of initial charts). If the referring physician's office is planning on faxing or mailing your records, please follow up with our office in advance to see if we have received the necessary documents.  

The following is a list of the typical records needed for the initial consultation:

  • Pathology report  from the biopsy

  • X-rays and/or scans (reports only) - Mammograms, X-rays, Ultra Sounds, CT scans

  • ER/PR (estrogen, progesterone receptors) Report (if breast cancer diagnosis)

  • Hospital discharge summary (if hospitalized)

  • Operative report (if surgery performed)

  • Pertinent laboratory results (especially if a hematology consult)

  • Radiation or chemotherapy reports (if treated in the past)

  • Current medication list

In addition to your records, on the day of your appointment, please bring a photo ID, your insurance cards, and your referral if required by your plan. You can also print out the PATIENT FORMS and fill them out prior to your appointment, if you choose.

What to expect at your first consultation appointment

  1. When making your appointment on the phone, the secretary will give you an appointment time that is 15 minutes before your scheduled time with the physician. This time will be used to meet with our Business Office to go over insurance matters, fill out paper work and copy your insurance cards.

  2. You will then proceed to the laboratory. The technician will get your weight and draw blood if necessary.

  3. Your consultation with the doctor will be approximately one hour to go over your medical history, to discuss your diagnosis in detail and to discuss treatment options.

  4. You will then check out at the front desk. The secretary will schedule upcoming appointments and provide you with any resources you will need. If your physician orders additional testing, please let the secretary know in case a preauthorization through your insurance company is necessary, which we will take care of.
